Step 1 - Become a Member and Gain Access
Each member is required to purchase a Key Code Access Plan prior to scheduling their first apptointment. Key code will be sent out once first appointment is scheduled. Must be 16 years of age to be in the facility without an adult.
Step 2 - Schedule your first Appt
You will get your code sent to you via email.
Step 3 - Sign a Waiver
Each athlete will need to sign a waiver prior to using the facility. You can download the file and send to lyjah@cgsportsmadison.com or sign one at the facility.
Step 4 - Utilize the facility
The facility is there for you to use. If there are any problems or issues contact Lyjah right away! Each rental does have security cameras in use. Any unlawful or non-ethical activities will be looked at and decided upon. If CG Sports feels the need to cancel your Key Code for these reasons we can at any time. We want a safe a secure facility that every athletes feels welcomed.
